HCP Mobile Physical Therapy is a home based private PT practice. This practice provides services in the comfort of your home including therapy with hospital to home transitions, supplementing home health, improving physical mobility limitations, and addressing your impairments to help you return to your optimal functioning level. HCP Mobile Physical Therapy was created to provide you one-on-one PT in a private setting while listening to your care and concerns without limitations of time, paperwork, or insurance restrictions. HCP Mobile Physical Therapy wants to help you meet your goals for mobility and help you live your best life.
Welcome
Hi, I’m Holly Passafiume PT, DPT. I graduated with my Doctorate degree in Physical Therapy from the University of Evansville in 2012 and became a Board-Certified Clinical Specialist in Geriatric Physical Therapy in 2019. My career has mostly revolved around working with the adult population in the hospital setting and skilled nursing facilities. After working in these settings, I realized I wanted to spend more time interacting with patients and focusing on their needs instead of worrying about the restrictions of insurance paperwork, time, and productivity standards. As a result, I created this home based physical therapy practice to provide services in the comfort of your home environment. I strive to provide caring one-on-one treatment to you while incorporating your individualized needs and goals. I recently moved to Raleigh in 2022 from the Louisville, KY area. I reside in Raleigh with my husband, daughter, and two dogs.
